New Patient Forms & Appointment Time
Chester County Dental Arts requires that new patients complete your new patient forms from the comfort of home prior to your appointment. If you have any questions regarding the forms, kindly ask the front desk staff when you arrive for your appointment. We ask that all patients arrive 15 minutes prior to their scheduled appointment time to ensure all paperwork, insurance questions and deposits are taken prior to treatment.
